Welcome to Dentistry by Design. These Terms of Service explain how we provide care and communicate with our patients. By receiving treatment or using our communication systems, you agree to these Terms.
1. Services
We provide comprehensive dental care in accordance with Missouri law, including preventive, restorative, and cosmetic treatments. All care is delivered by licensed professionals.
2. Appointments and Cancellations
• Please provide at least 24 hours’ notice if you need to reschedule or cancel.
• Missed or late-cancelled appointments may be subject to a $50 fee.
3. Payment and Insurance
• Payment is due at the time of service unless prior arrangements have been made.
• We accept most major insurance plans and will assist with claim submissions.
• You are ultimately responsible for any charges not covered by insurance.
4. SMS / Text Messaging Disclosure
By providing your mobile phone number, you consent to receive text messages from Dentistry by Design for appointment reminders, marketing messages, and general two-way communication.

5. Electronic Communications

Dentistry by Design is not responsible for any charges, errors, or delays in SMS delivery caused by your
carrier or third-party service providers. By opting in, you confirm that you are the owner or authorized
user of the phone number provided and that you are at least 18 years old.
7. Changes to These Terms
We may update these Terms occasionally. The most current version will always be available in our office
or on our website. Continued use of our services means you accept any updates.
